Module 14—Patient Rights and Responsibilities

Transferring to another clinic: Responsibilities

If you are on dialysis and need to transfer to another dialysis center, it's your job to talk with your social worker about your options. He or she can tell you about other centers in your area. You can also look for a new center yourself. There are a few ways you can find a new center if you're not happy with your current one, if you're moving to a new place, or if you'd like to travel:

At www.globaldialysis.com, you can look for centers in the U.S. or around the world.
To find a clinic that offers home dialysis (PD or home hemo), go to the "Find a Center" database at Home Dialysis Central.
Once you find a center, go to Medicare's Dialysis Facility Compare site to learn how it compares to other nearby centers for dialysis adequacy, anemia control, and patient survival, what types of dialysis they offer, and more.
